Medication Policy
Wylie ISD Guidelines for Administering Medication in School
Recognizing there are occasions when it is necessary for the school to administer medication to students during school hours, the following guidelines have been established:

1.Written request from the parent or legal guardian should accompany all
prescription and OTC medications and state the following: student's name, date of permission, number of days to be given, time of day to be given, signature of parent or legal guardian.

INHALERS- Any student that needs to carry their inhaler must have a physicians's order and a request from the parent or guardian.

2. Prescription medication brought to school must be in the original container (NO BAGGIES) dispensed by a pharmacy or doctor. The container label should state the patient's name, date , name of medication , dosage , and time to be given.

3.Over the counter medication must be in its original container and age appropriate.

4. All sample medications from a doctor's office must be accompanied by a note from the physician

5.Change in original request must be done in writing for prescription medications from the physician. Faxed orders are accepted. 325-695-4647

Suggestions:
1.Please ask your pharmacist to make a second labeled prescription bottle for medication to be taken at school.
2.Medication that is to be given three times a day may be given before school, right after school and at bedtime. (unless a physician specifies a definite time schedule).
